Waiver:  Student acknowledged the existence of certain risks inherent in any health or martial arts training and hereby agrees to absolve Kam Lee Center - Taiji Kung Fu Academy (TKA), its staff and management, fellow students and property owners (East West Condominium, Health Wisdom Inc.) from all responsibilities and liabilities arising from personal injury or property.  Student further agrees and allows the use of their verbal comments, pictures, surveillance cameras or videos taken at TKA classes or events for school promotions including, but not limited to advertising in brochures, flyers, videos and/or for websites marketing materials, etc.

Representation:  Student hereby represents that he or she is physically fit to participate in TKA’s health and/or martial arts training and that this is the entire agreement of both parties, and that no oral or written representations have been made to student except those expressly stated in this Agreement.  Student represents that he or she has read this Agreement in its entirety, understands its contents and agrees to its terms and conditions.

Government Law:  This Agreement shall be construed, deem legal and governed by the state laws of Florida, USA.
